UIL Physical Form and Acknowledgement of Rules
This form must be completed annually for student to participate in athletic activities. After completing the first three pages online, please print out all four pages to take to your physical exam. The doctor will complete the fourth page at the exam.

Extracurricular and Co-curricular Pledge
Students who participate in extracurricular or co-curricular activities must sign a pledge to refrain from using illegal substances or being in a place where they are being used.
