5^th Grade Discipline Plan

• Follow directions the 1^st time
• Keep your hands, feet & objects to yourself
• Talk at appropriate times
• Respect yourself, others & property
• Accept responsibility for your actions
• Come to class prepared, including materials and homework (This is counted towards the Work Study Grade)

Consequences

1 offense= Warning

2 offense = no recess

3 offense = parent contact (note, phone, person, or email) and no recess

4 offense = teacher/parent contract, or office referral

*** NOTE: Severe offenses receive immediate referrals!

Conduct Grade

Conduct grades will be determined by the number of offenses the student receives in that 9 weeks.

E = 0 – 3 offenses

S = 4 – 6 offenses

N = 7 – 9 offenses

U = 10 or more offenses

Rewards

• Students receiving fewer than 3 offenses in a 9 weeks could earn any of the following:
• Extra computer time
• Extra recess
• Sitting with friends during lunch or programs
• Homework pass
• Teachers choice
