Volunteer Information Release Forms
For the protection of students and staff, all applicants for volunteer work within the district are asked to complete a form authorizing the district to obtain a criminal history background check. A new form must be filled out and submitted each year. Forms are available on this website in Adobe Acrobat format and may be printed out, completed and returned to a campus.
Forms should be sent in as soon as possible so that all necessary paperwork may be ready at the begining of the school year.
