Our Colleyville Middle School nurse provides student-centered health services. It is her aim to assure a safe, healthy environment that is conducive to learning and to provide professional care for those who become ill or injured while at school. She also works to ensure that immunization requirements are met, and provides vision, hearing, spinal, and Acanthosis Nigricans screenings and referrals in accordance with State Law and Regulations.
Clinic
The clinic is designed to treat minor injuries and to act as a temporary "waiting area" for students who have become ill during the school day. Should a student become ill, persons designated on the enrollment card will be contacted to pick up the student. All telephone numbers must be recorded on the "Health Information and Emergency Consent" card located in the nurse's office, and updated immediately if changes occur during the school year.
Students will be sent home and should stay home from school due to illness if any of the following are present:
- fever of 100 degrees or higher in the past 24 hours;
- itchy, red eyes with discharge;
- unknown rash;
- ringworm of the scalp (until treatment has been started);
- head lice or nits
- jaundice.

Medication Procedures
All medication should be given outside of school hours if possible. Only medication that is required to enable a student to stay in school will be given, under the following conditions:
- All student medication must be kept in the clinic. Medications must be in the original prescription or non-prescription container. The pharmacy will supply two (2) labeled bottles for this purpose. Medications will not be given if sent in baggies or unlabeled containers. Written permission of the parent or guardian is required for the administration of all medications. Medication permission forms may be obtained from the nurse's office or on this site. New permission forms are required each school year. If the medication will be given longer than 10 days, we are required to have a physician signature on the medication form along with the parent signature. If you wish to fax us or have your physician fax us the permission form, our fax number is 817-498-9764. Ritalin and other controlled medications will require a physician's signature as well as the parent/guardian signature. Medication samples must be accompanied by a written prescription
- For students who need to carry their inhalers at school, please complete the "Authorization to Carry A Prescription Inhaler Form". We request that a second inhaler be kept in the health room in the event that the student forgets the inhaler at home. All inhalers must have a label on the inhaler or be sent with the box prescription label. Inhalers may be kept in the clinic with only a parent permission on file.
